New patients must be at least 18 years old with current medical insurance. We accept most, but not all insurance plans. For more information, please click on the plus sign next to the statement that fits you.

  • Currently Dr Burkett is accepting new Medicare patients.

    He can accept regular Medicare as well as: Blue Advantage, Humana Gold Choice, UHC Medicare Complete and Viva Medicare Plus

    If you have a different policy, we will not be able to see you.

  • While we accept many Blue Cross plans, we do not accept the BEG or PGX contracts available on the marketplace exchange. While the insurance directory may list us as doctors on their plan, we may not be accepting that plan just now.

    If you have a BCBS plan other than from the marketplace, please complete the application at the bottom of this page.

  • We do accept Viva plans, including the Viva Medicare Plus plan. Please complete the online application at the bottom of this page.

  • We do accept some UHC plans and we must check eligibility plan by plan; please do allow extra time for our approval process. While the insurance directory may list us as doctors on their plan, we may not be accepting that plan just now.

  • We do accept Aetna and Cigna regular commercial plans, but do not accept their Senior or Medicare supplement plans.

    If you have a non-Medicare plan, please complete the online application at the bottom of the page.

  • We can accept Tricare Select, but will be out of network, leaving you a larger portion of co-insurance.

    We can not accept Medicaid, Merit or other Tricare plans. However, Tricare For Life can be filed as a secondary.

  • If you have an insurance plan not listed here, please complete the online application at the bottom of this page and we will check into availability for you.

  • Dr Burkett is accepting patients older than 54. Please complete the online application at the bottom of this page.

  • We do not see new patients for emergencies, urgent sick visits, worker compensation injuries or for treatment after a motor vehicle accident.   If you have an urgent need, please seek help at an urgent care center or emergency room.

    Due to demand, the process of becoming a new patient could take up to four weeks or longer depending on our ability to reach you by phone. We will try to make allowances for acute needs. Our New Patient Coordinator will call you when she receives your application. Please provide a good call back number.

    In-office staff cannot speed up this process - we do ask you to be patient and wait on the New patient Coordinator to call.

Please allow up to two weeks or more for a response. Demand is currently high and the process is dependant on our ability to reach you by phone. The application process is not handled by staff at the practice, so you will need to wait for the outside service to call you. Please make sure they have a good contact number.

Once accepted as a new patient, we will ask you to establish an account in our Patient Portal so that we may create a record for you.   Patient Agreement forms and family health history will be securely submitted to your new chart.  We do require all new patients to use online means for form completion and receiving results.  While we realize the initial set up will require time, it will improve the delivery of healthcare data over the long run. 

For you first appointment, please bring:

  • medical information and medications from your previous physician,

  • your insurance card,

  • your driver's license or ID, and

  • for patient 25 years and under, please bring a copy of your vaccination record.
