Forms
Forms requiring completion by the practice or needing a doctor’s signature (such as Wellness forms, FMLA forms or applications for assistance) should be brought in during a scheduled visit. If that is not possible, bring them to our receptionists who can explain the process for completing them. We apologize, but do not keep or provide third-party forms here.
Many forms require specific information and must be evaluated before we can committ to completing them. A committment to complete a form can not be made over the phone; form requests must be processed by a receptionist or during a scheduled visit.
Forms can take up to ten days to complete. There is a fee for all forms completed outside a scheduled visit. To avoid this fee, please remember to bring your needed wellness or clearance forms to your visits. (We do not provide forms) Some complex forms require a charge regardless of a visit.
